Application Procedure

The Prospectus & Admission Form are available at Cash counter of the Institute on payment of Rs. 500/- (Rupees Five Hundred only.) The Admission Form should also be submitted online through our website www.bareillyparamedical.com
The application form should be duly filled accompanied by attested photo copies of the certificate required. Incomplete form will be rejected without intimation. The complete application form should be submitted in the office by hand or by post or through online to the institute by the specified date. No forms will be accepted after the due date. The application form is common for all the courses. The candidate filling the form should also mention clearly the choice of the course according to the preference.

LIST OF ENCLOSURES:.
             Candidates are required to attach the following documents with properly filled application form duly self attested or by Gazetted Officer/Head of the Institute last              attended.
*           Attested photocopy of High School Marksheet.
*           Attested photocopy of High School Certificate for proof of Date of Birth.
*           Attested photocopy of Intermediate Marksheet.
*           Character Certificate by a Gazetted Officer.
*           Passport size coloured photograph affixed on the application form.
*           Attested photocopy of Aadhar card
